On the lower part, a semicircular tower of a fortress with a crenelled upper part and a fortification shaped as parallel horizontal bars; in the middle, the great signet of Prince Ştefan cel Mare, bearing Moldavia's Coat of Arms; above, Romania's Coat of Arms, the inscription "ROMANIA", the year of issue "2004" and the face value "5000 LEI" that is placed between two floral elements.
Reverse
The portrait of Ştefan cel Mare; behind, the name and image of the church of Putna monastery where the Prince is buried; the inscription "500 ANI DE LA MOARTEA LUI STEFAN CEL MARE" is engraved circularly; next to the portrait, the years "1504" and "2004".
